Support for Transition Planning

Change.  Moving up a grade, starting college, going to vocational training:  all are important life changes that can be particularly challenging to children with special needs. Transition planning puts parents and children at the threshold of the unknown.  That is when the support and experience of those who have been there before is especially valuable.

Smooth Transitions

HOPE provides support to parents and children with special needs before, during, and after transitions are made.

Our goal in transition planning involves assuring students that they are building on their success and entering a new and progressive phase of their education. Drawing on our own experience and professional backgrounds, we can develop a plan to bring a sense of continuity to whatever transition may be ahead.

Communication and Change

When change at school is on the horizon, communication is more important than ever. HOPE can work with parents, administrators, teachers, and students to identify hurdles that may be ahead and to discuss and implement intervention strategies.
